Reporting an Incident to the Office of Campus Information Security Office (OCIS)

In general, you can report any incident to OCIS that you feel is a violation of the UW-Madison Responsible Use of Information Technology Policy or is an information security issue involving a subject at the University of Wisconsin-Madison.

Per campus policy, if you are reporting the loss of University Sensitive data, please do not use this form and instead call the DoIT Help Desk at 608-264-HELP (4357).

When filing a report with OCIS please review the below points:

  • If the report involves electronic harassment and you fear for your physical safety, please contact your local police department immediately.
  • In general, submitted complaints cannot be withdrawn at a later date.
  • Time may be critical, thus please submit the report as soon as possible.
